Division of Geriatric Medicine

Locations

The Hamilton Wentworth Regional Geriatric Medicine Programme includes Medical and Psychiatric Services for the elderly in acute hospital, ambulatory care and long-term care facilities. In addition, each acute care site offers Geriatric Medicine consult teams.

At the Hamilton General (General) site of the Hamilton Health Sciences Corporation (HHS) there is a consultation service with a senior's speciality RN who works with a Geriatrician

At the Juravinski Hospital and Cancer Centre campus of HHS (Henderson), there is a 14 bed Complex Medicine Rehab Unit staffed by Geriatricians in rotation. There is also an active consultation service with a senior's speciality RN who works with the Geriatrician.

St. Joseph's Heathcare (SJH) offers clinics and a Geriatric Medicine consultation team. At the St. Joseph's Hospital Centre for Ambulatory Health Services (CAHS) there is an out-patient Geriatric Medicine Clinic, Osteoporosis clinic, a Day Hospital and Community Outreach Teams.

St. Peter's Hospital (now affiliated with HHS) has a Memory Clinic which offers multidisciplinary consultations to referring physicians. The Centre for Healthy Aging has four Out-Patient Geriatric Medicine (GMED) Specialty Clinics, and an Osteoporosis clinic. Psychiatry & Medicine for the Aged in the Community (PMAC) is an Outreach Team staffed by a Geriatricans that work closely with Geriatric Psychiatry (and which does home visits when indicated); all offer multidisciplinary consultations to referring physicians.
