The information Management Group (Jackie, Maria
and Ted) have developed templates and databases for various AIDS organizations
to help them with data collection, specifically for the Ministry of
Health and Long-Term Care quarterly reports. Some have been pre tested
in designated agencies and have received very positive feedback. |

The
first template is called "Phone
calls and e-mails". It is set-up so you can keep track
of your calls and e-mails for a month by entering a "1"
in the appropriate box on the computer form. It keeps a record of
the number of phone calls and e-mails under the categories used
on the MOHTLTC quarterly report. The form automatically totals these
for the month. These totals will help you complete the MOHLTC quarterly
report on page 2, question 2c. To view this template, please click

The
second template is called
"Volunteer Hours". It is set-up so volunteers can
keep track of their hours under Ministry headings and send them
in via e-mail. These hours, will help you complete the MOHLTC quarterly
report on page 3, question 3b. To view this template, please click

The
third template is called "Visits
and Calls". A receptionist at an organization would
use this to keep track of visits and calls that are coming in to
the agency and to whom they were directed. You would use on for
each month. To view this template, please click on Visits
